Your First Appointment
Please bring with you:
- GP/specialist referral
- Every patient booking requires a written referral from a GP or specialist
- Referrals can be faxed or emailed to the practice for your convenience- Any imaging including Xrays, CTs, MRIs, or other scans
- Medicare & Health fund cards
- Any other relevant medical information
Returning patients: please inform our reception staff on arrival if any of your details have changed.
To save time:
You have the option of downloading and completing your Patient Registration forms and Foot & Ankle Research surveys ahead of time. These can be emailed to our rooms. Alternatively, please arrive for your appointment at least 15 minutes early and our staff can help you complete the forms.
